Official Transcripts
• Official transcripts from all college-level institutions attended must be sent directly to the MPH Program Office. The address is as follows:

• Students who are applying to or are currently enrolled in a UW graduate or professional degree program may request that copies of their transcripts be sent from their home departments to the MPH Program Office.
• Students who are currently enrolled in an approved MPH dual degree program are waived from the transcript requirement.
• International applicants must have their transcripts evaluated by a NACES member credential evaluation service.

Resume
Your resume should include contact information, education, employment, honors/awards, publications, and memberships. Upload your resume in PDF format to the online application.
Personal Statement:
A personal statement, limited to one page (single-spaced), should address your prior public health experience, reasons for pursuing an MPH degree, and future public health goals.
You are encouraged to comment on any or all of the following: plans to use your education and training in public health; the needs and/or challenges you perceive as important in the field of public health; and any personal qualities, characteristics, and skills you possess will enable you to be successful as a public health practitioner or add to the diversity of the educational experience. The personal statement must be in PDF format in order to upload to the application.
Recommendation Letter:
Please provide the names and email addresses of three people from whom you would like to request letters of recommendation. Once you fill in the names of your references, they will receive an email from the MPH program asking them to upload their recommendation letter directly to the application.
• Letters should focus on your job performance accomplishments in the field of public health or potential for success in academic public health.
• Letters prepared for other academic or professional programs are strongly discouraged.
• Letters of recommendation should be less than one year old.
• Letters of recommendation must be submitted electronically by the writer and be in PDF format.
Under the Family Education Rights and Privacy Act of 1974 (Buckley Amendment), applicants are entitled to review letters of recommendation or to waive their right to access. If you wish to view a letter of recommendation, check the box below the letter writer’s name in the Reference section of the application. Your decision to view letters of recommendation does not impact the Admissions Committee decision.

课程要求:successfully completed at least one semester of college level mathematics that includes algebra and/or calculus (equivalent to MATH 112 at UW-Madison) or a college level statistics (STATS 201). In addition, candidates must have taken at minimum, one semester of a college level science course such as biology, chemistry, or physics.
